=Part 1= This section is fairly straightforward. *The name of the Political Party Endorsing your Candidacy is “Ontario Libertarian Party.” *Candidate’s Name as it will Appear on the Ballot – Usually just the first and last names, and my include nick-names, such as “Nick” even if it isn’t his legal name. *If you, the candidate, are going to take the Nomination Papers to the Returning Office, do not sign on page 1 until you are with the Returning Officer (RO). (It’s a good idea to take someone with you to witness your signature, in case the RO doesn’t want to provide a witness.) If someone other than the candidate is going to take the papers to the RO, then the candidate must sign in front of that person. He (or she) should not sign as the witness until he is with the RO. *The RO will sign the bottom of Page 1 as long as the form is complete. This does not confirm that you are on the Ballot. Someone in the RO will check the signatures. =Part 2 - Nomination by Electors= This is where you get the Electors (voters) to sign. *The Name of Candidate should be the name as it will appear on the ballot. Also fill in the name of the Electoral district. *Ask the elector to PRINT their name and address in the first two columns and sign in the third. Make sure you can read the name and address before they leave. *Whoever collects the signatures needs to complete and sign the “Witness to Signatures” section. (If two or more people are collecting signatures seperately, each will need a separate copy of Part 2.) Anyone, including the Candidate, may collect and sign for signatures. =Part 3 - Appointment of Designates = You can leave Part 3 blank. You, as the Candidate, may appoint people to be scrutineers or to attend the Official Tabulation of the vote, or you may attend yourself. Candidates for the major parties usually delegate this job to someone on their team. =Part 4 - Receipt for Candidate's Nomination Paper= Fill in Part 4 as indicated. If someone else is taking the Nomination papers to the RO then he is the “Person Filing Nomination.” Once the RO is satisfied that there are 25 valid signatures, she will sign Part 4 and give you the original page. =Part 5 - Notice of Withdrawal= Leave Part 5 blank. It would only need to be completed and submitted to the RO if the candidate decides to withdraw from the election.
